EMS Revenue Cycle White Paper | October 2026 Code Red in the Revenue Cycle: Pain Points and AI Solutions for EMS Providers
Rates are rising again, diesel is up $2.51 a gallon in a year, and more of every transport bill is landing on patients with less room to pay. This free report shows what's coming over the next 16 months, models three forecast scenarios for patient collections, and shows where EMS revenue teams still have control.
Four numbers every EMS revenue leader should know
A 16-month playbook for EMS billing and collections
Medicare raises lag costs by a year. Medicaid work requirements start in January 2027. The Medicare ground ambulance add-on payments expire December 31, 2027. This report puts every one of those dates on one timeline and shows what each does to your revenue cycle.

How a war overseas becomes an unpaid ambulance bill
When fuel, groceries and card payments rise together, the ambulance bill is the easiest one to set aside. There's no car to repossess and no service to shut off. The report traces each link, from oil prices to the patient's monthly budget.
Faster outreach wins in every scenario we tested
We modeled a base case, a prolonged conflict and an escalation stress test. With today's outreach, patient collections hold flat at best and fall 3 to 7 points if the conflict drags on. Providers that reach patients sooner still collect more than today's status quo in all three.
Collect more, faster, without adding overhead
Providers can't set their own Medicare rate or stop diesel from moving. They can collect more of what they've already earned, sooner, without hiring.
More touches, sooner
AI voice, text and email on every patient-owed account from the day the balance is final.
Inbound, 24/7
Patients who call back at night reach an agent that can explain the bill, set up a plan or take a payment.
We own the gateway
Payments run on our own gateway with DivDat, and 835s post back in real time or in batches.
Compliant by design
Separate workflows for consented and non-consented patients, built around TCPA, Regulation F and state rules.
Written by operators, not just analysts
EMS revenue cycle questions, answered
How much does a ground ambulance transport cost compared with what it's paid?
The first CMS Ground Ambulance Data Collection System report, prepared by RAND, put the mean cost of a ground transport at $2,673 against mean reimbursement of $1,147 across all payers. That's a $1,526 shortfall on every run, before any bad debt.
How will rising interest rates affect EMS patient collections in 2027?
The Federal Reserve raised rates on September 16, 2026 and its projections show no cuts through 2027. Higher rates raise card minimums and loan payments for patients, so an unsecured ambulance balance gets pushed to the back of the line. They also raise the cost of carrying AR on a provider's credit line.
How does the Iran war affect EMS providers?
The conflict that began February 28, 2026 sent oil prices sharply higher. U.S. diesel averaged $6.18 a gallon the week of October 12, 2026, up $2.51 in a year. Providers pay more to run their fleet, and patients have less room in their budgets to pay ambulance bills.
What Medicare changes hit ambulance providers in 2027?
The 2027 Ambulance Inflation Factor is projected at about 2.6%, below recent consumer inflation. The temporary 2% urban, 3% rural and 22.6% super-rural add-on payments run through December 31, 2027, then expire unless Congress acts again.
How can EMS providers collect patient balances faster without hiring?
AI agents can call, text and email every patient-owed account from the day the balance is final, answer inbound billing calls 24/7, and take payment in the same conversation. Evolve AI Agents from Intelligent Voice AI do this with separate compliant workflows for consented and non-consented patients and post 835s back to the billing system.
